Actually, it is four streets… Five points is located where the diagonal grid of the downtown streets meet up with the normal street grid where streets run east / west and north / south. The streets that meet up to make five points possible are: Welton Street, 26th Avenue, Washington street, and 27th Street. Welton Street has two of the five points.
